शिवलिंगं त्वया नाथ स्थापितं यन्ममोपरि । मम नाम्ना भवतु च कुशेश्वर इति स्मृतम्
śivaliṃgaṃ tvayā nātha sthāpitaṃ yanmamopari | mama nāmnā bhavatu ca kuśeśvara iti smṛtam
હે નાથ, તું મારા ઉપર જે શિવલિંગ સ્થાપિત કર્યું છે, તે મારા નામથી પ્રસિદ્ધ થાઓ અને ‘કુશેશ્વર’ તરીકે સ્મરિત થાઓ.
