शिवलिंगं त्वया नाथ स्थापितं यन्ममोपरि । मम नाम्ना भवतु च कुशेश्वर इति स्मृतम्
śivaliṃgaṃ tvayā nātha sthāpitaṃ yanmamopari | mama nāmnā bhavatu ca kuśeśvara iti smṛtam
हे नाथ, तू माझ्या वर जे शिवलिंग स्थापित केले आहे, ते माझ्या नावाने प्रसिद्ध होवो आणि ‘कुशेश्वर’ असे स्मरणात राहो.
