The Wheel of Time and the Norms of the Āśramas

The verse highlights how human suffering and limitation—old age, grief, illness, and adversity—manifest outwardly even as something as simple as a sound. Ethically, it encourages discernment and compassion: one should interpret signs of strain in others with understanding, recognizing the conditioned nature of distress (by time, place, and circumstance).
Vāyudeva is explaining the nature and cause of a particular sound/utterance, identifying it as the noise produced by fatigue and strenuous exertion, and tracing it to underlying conditions such as illness, misfortune, old age, and grief, which fluctuate with context (place and time).
